Annual Permit Application
New for 2012: Insurance will be required for all Annual Permits. On December 19, 2011, our office sent out a letter to Current Annual Permit holders notifying them of the new insurance requirements. We have received some phone calls from Annual Permit holders and/or their insurance agents in regards to the Automobile Liability insurance requirements. We have gone back to the State’s Risk Management Office for clarification. The determination is that Automobile Liability is required for Annual Permits. However, as some of these concerns/questions were unanticipated, we will be meeting with the State’s Risk Management Office and the State’s insurance provider to address these issues.
Given the need for further clarification, we will hold off on implementation of the insurance requirements for Automobile Liability for Annual Permits. If you currently have Automobile Liability, then the State will accept that coverage. However, we are going forward with the General Liability requirements as described below:
A Certificate of Insurance for $1 million general liability naming the State of Hawaii as additional insured, is required for Annual Film Permits.
REQUIRED Wording:
In the “Description of Operations/Locations/Vehicles/Special Items” section:
“The State of Hawaii, including all of its departments and attached agencies, their officers, employees and agents are named as additional insured with respect to the named insured’s filming activities.”
***Any deviation from the above wording may require a review by the State’s Risk Management Office and may delay the processing of your Annual Film Permit Application.***

The “insured” on the Certificate of Insurance must be the same as the “Applicant” on the Annual Permit application.
If you are having trouble obtaining the necessary General Liability insurance, do not have General Liability insurance, cannot provide the required wording, or have lower coverage than our minimum requirement, please email (info@hawaiifilmoffice.com) us and we will contact you to discuss your situation.
Our Annual Film Permit Application may be downloaded from the links below as a fillable PDF or fillable/saveable MS Word document. We suggest that you save these forms to your computer, fill them out on your computer, print them, and fax them to us for processing.
Permit applications require your signature before submission and must be faxed to the Hawaii Film Office at (808) 586-2572. To renew your expiring annual permit, simply fax us a new permit application. Permits expire every December 31st, so be sure to apply in time for renewal.
There are two types of Annual Permit badges:
1. Tour Group Photography:

Permitted in designated DLNR-administered park(s) named on your badge. Weekend and holiday filming is allowed at Nuuanu Pali State Wayside Park, but is highly discouraged at other sites, particularly those that are popular with the general public.

Badge must be worn at all times while on location.

No animals allowed on location.
2. Scenic/Stock, Weddings, Model Portfolios (film, video, stills):

Permitted at DLNR open/accessible sites that do not require a special permit. Weekend and holiday filming is highly discouraged, particularly at sites that are popular with the general public.

Not permitted in DLNR Boating-administered areas (Waikiki Beach--from beach fronting the Reef Hotel to the Moana Hotel, plus Ala Wai and other recreational harbors)

Badge must be worn at all times while on location.

For those companies that specialize in coordinating weddings, a DLNR Special Use Permit is required for weddings held at DLNR State Parks administered areas, and a DLNR Right-Of-Entry Permit is required for weddings held at DLNR Land Management administered areas.

Photography for calendars, class pictures, fashion catalogs/magazines, travel brochures, and other publications, are not covered under an annual permit and require a standard Film Permit Application. The Annual Permit is subject to the General Conditions for use of State agency-administered areas.
