Certificate of Insurance
A Certificate of Insurance for $1
million general liability and $1 million combined
single limit automobile liability (for owned, non-owned and
hired vehicles), naming the State of Hawaii as additional
insured, is required for Standard and Pre-Approved Film
Permits.
In the "Description of Operations/Locations/Vehicles/Special Items" section:
"The State of Hawaii, including all of its departments and attached agencies, their officers, employees and agents are named as additional insured with respect to the named insured’s filming activities."
In the "Certificate Holder" section:

- The "insured" on the Certificate of Insurance must be the same as
the "Applicant" on the film permit applications.
- The University of Hawaii must also be named as additionally insured when filming at any of its facilities.
- Kaka'ako Waterfront Park is no longer an Open and Accessible Site on Oahu. The Hawaii Community Development Authority (HCDA) must also be named as additionally insured when filming at Kaka'ako Waterfront Park.
A copy of the Certificate of Insurance must be faxed to the Hawaii Film Office with your Film Permit Application. The original Certificate must also be emailed to us in PDF format to info@hawaiifilmoffice.com or mailed to us at: Hawaii Film Office, PO Box 2359, Honolulu, HI 96804.
